Educators - Creating a New File

To create a NEW reference file, please submit the following:

  • Electronic Sign Up for Services Form
    (If you've already submitted this form electronically, please do not try to re-submit. If you experience problems with this form, please call 360.650.2980.)

  • Payment Options Form
    (Please print and return this form with your $30 fee.)

  • Declaration of Option Form
    (This form lets us know whether you want to establish a confidential or non-confidential file. Information to help you make your decision is included with the form.)

  • Candidate Information Form
    (The information from this form will be the first page of your reference letter file. Please remember to submit a new form if your contact information changes.)

  • Letters of Reference
    (Please refer to the FAQ page for additional information.)

To RENEW/UPDATE an existing reference file, please submit the following:

  • Registration Form to Update Services
    (Payment accompanies this form and includes access to the file service as well as career counseling appointments.)

  • Candidate Information Form
