To create a NEW placement file, please submit the following:
- Electronic Sign Up for Services Form

(Please print and return this form with your $30 fee.) - Declaration of Option Form
(This form lets us know whether you want to establish a confidential or non-confidential file. Information to help you make your decision is included with the form.) - Candidate Information Form
