I'm worried about a friend. He has pain in his left lower quadrant, has been puking up blood, and has heartburn. Most of this occurs when he drinks. He scared so he doesn't want to go to a doctor. What could be wrong with him and what should I do?

 

A:

Obviously these are very concerning symptoms that may represent alcohol related ulcer or gastritis/esophagitis, and any bleeding from the stomach or gut is potentially serious and life threatening. The pain in the left lower abdomen may not necessarily be related but needs evaluation as well. Offer to go along with him to the clinic as a support person, to listen to the health care provider's assessment and to help your friend articulate his fears and concerns. It is also important to talk about why your friend continues to drink alcohol, even though it results in such unpleasant and serious side effects.
