I received an HPV vaccine last week, and forgot to fill out an envelope to have the records/receipt mailed to me for insurance reasons. Is there anyway I can still request that information? Can I just call, or should I show up and talk to the receptionist in person? Thank you!

You can call 650-3400 and leave a message on our message line (one of the options given) clearly stating the name and address you want the billing information sent to for insurance reimbursement. We can't send it directly to an insurance company but can send it to someone (like your parents) who can forward it to insurance company.
