I've had generalized anxiety disorder for a few years now and I see a doctor near my home but just recently I've missed a class due to intense anxiety attacks. Usually, I'm able to handle myself and not miss class, or if I do miss class it's always one that doesn't take attendance so it doesn't matter. But recently I missed a class that I need a doctor's note to be excused from or else I miss points off my grade. I have a medical reason for not being in class so how do I get a note from a doctor. I don't want to go through the trouble of trying to get in touch with my doctor at home as he is very far away. Thanks.

There are student verification of illness notes available at our front desk receptionist that you can fill out yourself, without being seen by a provider here, but it is not a doctor's note as we can't verify an illness we haven't treated or diagnosed. Most instructors accept that form for a class absence but some don't. You will need to give permission for the instructor to contact us about whether or not you were seen for the illness that kept you out of class.

If you prefer to be seen for a doctor's note, you can request a same day appointment by calling 650-3400 and a provider can see you and provide a note, but that would involve getting your previous records for some background and history on your anxiety disorder.
