The Consulate of Chile requires Health Exams signed by a physician (Not an RN or PA). Do I need to sign up for a physical appointment and have tests or measurements done or can I hand over the documents that need to be signed?
A:
A complete history and physical exam is required to fill out the form for the Consulate. We have no more travel appointment availability this quarter so unless you are a summer quarter student, you'll need to get it done through a walk in clinic in the community.
