Required Forms

Prior to your initial consultation appointment, please complete the Counseling Center forms. You can do this at your convenience by following 3 easy steps.

1. Print them out. (You will need Adobe Acrobat Reader to view PDF files.)
2. Complete them in pen.
3. Bring them to the Counseling Center for your initial consultation. Please arrive at least 10 minutes early to turn in your paperwork.

If you prefer to complete the forms in our waiting room, please come to the Counseling Center at least 20 minutes before your appointment, and we will provide you with a hard copy of the forms.
