Notice of Privacy Practices

The WWU Student Health Center, in compliance with the Health Insurance Portability and Accountability Act (HIPAA), provides this Notice of Privacy Practices to all students accessing our services, and must have written acknowledgement by signature that the Notice has been received.

As one of our patients, we encourage you to read this document prior to coming to clinic. On your next visit to the Health Center, you will be asked to sign a form for your medical chart that you have received this information.
