Order Course Evaluations

NOTE: Deadline for ordering Fall 2009 evaluation packets is November 13!

The Testing Center provides:

• Scannable evaluation packets seven different class formats;
• An online evaluation process designed specifically for distance learning courses; and
• A customizable Midterm Evaluation feedback form embedded in all Blackboard courses for use at any time for private, current, actionable feedback to help you improve student learning.

Ordering Standard In-Class Evaluation Packets

Use the "Log in" button below to link to Faculty Web for ordering paper-and-pencil course evaluation packets for any of the seven evaluation forms available for standard on-campus classes. Each form has a slightly different set of questions designed to meet the needs of different class formats, as well as the option to ask up to five customized questions of your own design.

Ordering Online Distance Learning Evaluations

To order ONLY evaluations for online or distance learning classes (which students access and take online), do not use the button below, but instead use the WWU Course Evaluation Order Form For Online Courses. After you submit the order form you should receive an email (which you should save) with the URL where your students can find the evaluation, and another where you can monitor the number of submissions so you can send students reminders as necessary.

Suggestions

Before logging on to order evaluations, it is recommended that you:

• Decide which class format/evaluation form you wish to use for each class; (Click to view options)
• Know your Western ID and PIN* (usually first six digits of SSN);
• Become familiar with the detailed instructions for using the online ordering system: (pdf format) or (Word format)
• Decide whether you want to allow your evaluation results to be posted in the public archive (default choice), or if you want results kept private, available only to you.

TIPS: When logging on to Faculty Web:
• Exclude any suffix numbers when entering ID to log on;
• For logon help, call ext. 2853;
• Once logged on, repeat ordering procedures for each class you want to evaluate.
