Student Evaluation Form Options

Under the direction of the Faculty Senate, the Testing Center offers faculty seven different teaching evaluation forms, each tailored to the needs of different instructional settings. Descriptions of the form options and the questions on each form are available below to help you select the form that is best for each of your classes.

Note: Although each form was designed with particular class formats in mind, faculty are invited to base their form selections on the appropriateness of the items for their courses and objectives.
