Student Evaluation Form Options
Under the direction of the Faculty Senate, the Testing Center
offers faculty seven different teaching evaluation forms, each
tailored to the needs of different instructional settings. Descriptions
of the form options and the questions on each form are available
below to help you select the form that is best for each of your
classes.
Note: Although each form was designed with particular class
formats in mind, faculty are invited to base their form selections
on the appropriateness of the items for their courses and objectives.

Form A: Small lecture class
Designed for small lecture/discussion courses. Items emphasize
the clarity and quality of information transmitted, as well
as the nature of the interaction between instructor and student.
Form B: Large lecture class
Designed for large lecture classes, with little or no in-class
interaction between instructor and student. Items strongly emphasize
the quality of course organization and information transmitted.
Form C: Seminar or Discussion Group
Designed for seminar or discussion classes which include
a minimal amount of formal lecturing by the instructor. Items
emphasize quality of discussion as well as course organization
and interest level.
Form D: Problem Solving Class
Designed for those classes whose purpose is the teaching
of problem-solving or heuristic methods. Clear explanations
dealing with student difficulties and quality of problems
are emphasized.
Form E: Skill Acquisition Class
Designed for those classes which are skill oriented and in
which students get "hands-on" experiences related
to future occupational demands.
Form F: Large Lecture/Homework
Designed for use in large lecture classes which rely
heavily on homework problems and a textbook. Emphasis
is on instructor's ability to communicate with students,
and the value of assigned problems and readings.
Form G: Lab section
Designed for lab sections generally taught in conjuction
with classes in the physical sciences. Items emphasize
the instructor's ability to introduce meaningful questions,
assist students, and deal with unexpected problems.
Form H: Distance Learning (online)
Designed for online distance learning courses where instructor
and students generally do not meet face to face. Items emphasize
the instructor's ability to creative an online learning environment
and community. Student comments, written on separate sheets,
and a computerized summary report will be returned to instructors
for each class surveyed.
