Procedure 8: Requesting Other Services or Accommodations
(Effective August 27, 2001).

After disAbility Resources for Students (DRS) completes a Needs Assessment for the student and determines that the student is qualified to receive services, it is the student's responsibility to request those services. The following procedure is designed for a student to request those services or accommodations that may be unique to his or her academic commitment.

1. Action by Student: Contacts the Director of DRS or designee to discuss accommodation needs.

2. Action by Director of DRS or Designee: Ascertains the student's needs and accommodation options.

3. Action by Student: Provides additional documentation to support the accommodation, if requested.

4. Action by Director of DRS or Designee: Approves the request to provide the accommodation or denies the request.
