1. Action by Student: Completes an Exam Accommodation Request Form and returns it to DRS within ten business days after the first day of classes.
2. Action by DRS: Creates accommodation letters for the student's instructors.
3. Action by Student:
- Checks back with DRS no earlier than two business days after submitting the Exam Accommodation Request Form to get accommodation notification letters.
- Hand delivers the accommodation notification letters to each professor.
- Picks up Proctored Exam Form from DRS.
- Completes Section 1 on the Proctored Exam Form.
- Hand delivers the Proctored Exam Form to each instructor who will be administering tests to the student that quarter.
- Asks the instructor to complete Section 2 and returns the form to the student.
5. Action by Student:
- Hand delivers the Proctored Exam Form to DRS no later than five business days before the exam is to take place.
- Arrives at DRS or the prearranged testing location on the scheduled day and time of the exam.
- Completes the exam.
- Returns the completed exam to DRS personnel or designated testing official.
- Students are required to complete only one Exam Accommodation Request Form at the beginning of each quarter. This form is valid for all exam accommodation requests that the student makes during the quarter.
- Students who attend classes outside of the University's main campus in Bellingham may arrange to receive and deliver materials to the main campus via fax or mail instead of handling them in person.
