The Request for Public Records form is available as a printable online form by clicking here, or you may request the form by contacting the WWU Library Administration Office at (360) 650-3051.

Procedures

Complete Section I and II of the form. Please be specific in describing the records you want to see.

Submit the form either by mail, fax or in person to the address noted at the bottom of the request form. University Public Records Officer, Haggard Hall 231, WWU, Bellingham, WA 98225-9103, fax (360) 650-3044.  The form cannot be submitted electronically.

Within five business days of receiving a public record request the agency must respond by either (1) providing the record; (2) acknowledging that the agency has received the request and providing a reasonable estimate of the time the agency will require to respond to the request; or (3) denying the public record request.

Important Information

Determination will be made whether the requested information is allowable and if so what department will handle the request.

The University Public Records Office will coordinate the release of the information.

The University may impose a charge for providing copies of public records and for the use by any person of agency equipment to copy public records. No person shall be released a record which has been copied until and unless the person requesting the copied public record has tendered payment for such copying to the records official from whom the public record was obtained, or to any person designated by such records official.
