• Activity Form

  Please send to Tracey Finch, MS 9038 AS SOON AS POSSIBLE. Please use one form for each activity. The information on the form will be used in the Schedule of Events.

• Banquet Nomination Form

  Forms are accepted from approved departmental coordinators who turned in Activity Forms. Please return, either in paper format (use the print button on the form) or as an email attachment (use the submit by email button on form) to Tracey Finch, MS 9038 or email to scholars.week@wwu.edu by April 24, 2008. Please use additional forms if necessary. It is the intention of the committee to include all student presenters and their mentors.
