Important Note

The Benefits Office is required to act on court and medical support orders by an issuing agency such as the Department of Social and Health Services (DSHS). Upon receipt of the court or medical support order, we will proceed to enroll the employee and/or dependent(s) listed in the order in a medical and dental plan regardless of whether the employee has received a copy of the order or submitted the forms listed above.
