Eco Rep Forms: Event Evaluation
Submit this form if you are an Eco Rep or student hosting a program in Western's residence halls focused on sustainability. Only programs that submit both planning and evaluation sheets will be given points for the Go for the Green Competition.
This form must be submitted no later than 2 weeks after your program date in order to receive points.
After submitting this form, please send digital copies of pictures to ResRAPAsst@wwu.edu.
